COVID Waiver – Wilmington Kennel Club

A COVID Waiver must be signed before you attend a class or event at WKC.

CLASSES: If the waiver was part of your registration, you do not need to fill out this form. If you have not yet signed the waiver or are not sure, submit the form below to complete the requirement for attendance.

I fully attest to the best of my knowledge in the event that I have COVID-19, I will not attend any events, classes, meetings, and/or gatherings at WKC. I will not attend any of these events if I have been in contact with or exposed to any known carrier of COVID -19 within the fourteen days prior to the events, classes, meetings and or gatherings at WKC. I agree that I am attending any of these events, classes, meetings and or gatherings entirely at my own risk and take full responsibility for my own and my pet's health and safety during these events. I will follow all American Kennel Club (AKC), WKC clubs’ rules, requirements, procedures, protocols and guidelines to reduce any exposure and possibility of contracting or spreading the virus. I will also follow CDC, Federal, State of Delaware, New Castle County and AVMA guidelines regarding COVID-19. I fully submit that the WKC members or volunteers are in no way liable for any present or future COVID-19 exposure incurred at any time by any person, in attendance or not in attendance, during or after this event, and hereby waive all rights to file a lawsuit against WKC and the members if I am exposed to COVID-19. By signing this waiver below, I hereby agree to follow everything contained within this waiver.
